Forms let you build custom checklists, questionnaires, and intake forms that your team fills out on jobs, requests, and service plan appointments. You can add different field types, mark fields as required, and track which forms have been completed before work is closed out.
Head to the Forms section in the left sidebar of your dashboard. Click the New Form button in the top right corner. This creates a blank form that you can start building right away.
Give your form a name at the top of the page. You can also add a description and instructions. Instructions are shown to whoever fills out the form, so use this space for any context or guidance they might need.
Click the Add Field dropdown below your existing fields to add a new one. BlueSuite supports the following field types:

You can drag and drop fields to reorder them. When you’re done, click Save to keep your changes.
Open a job and scroll to the Forms section. Click the gear icon to open form settings. Here you can select which forms should be attached to the job.
For each form you attach, you have four options:
You can set default forms in your workspace settings so that every new job automatically has certain forms attached. Go to Settings > Jobs & Operations > Jobs and configure your default job forms and default appointment forms there.
Forms can also be attached to requests. Open a request and look for the Forms section. Click the gear icon to open form settings, then select which forms to attach.
For each form you attach to a request, you can choose where it applies:
You can attach the same form to both the request and its appointments if needed. For example, you might want a checklist filled out once at the request level and a separate inspection form completed at each appointment.
Go to Settings > Marketing > Requests to set up default forms for requests across your workspace. You can configure two types of defaults:
Service plans support attaching forms to the appointments that get created for subscriptions. Open a service plan and look for the Appointment Forms section. Click the gear icon to select which forms to attach.
Every appointment created for subscriptions on that service plan will have the selected forms available. For each form, you can mark it as Required so that the appointment cannot be completed until the form is filled out.
This is useful for recurring service work where your team needs to complete the same checklist or inspection form at every visit.
On a job, request, or appointment page, you’ll see each attached form listed with a status badge. Click on a form to open it and fill in the fields.
Fill in the required fields and any optional ones, then click Submit. The form status will change from Pending to Completed.
If you need to make changes after submitting, click on the form again to view your submission. Click Edit to update your answers, or delete the submission to start over.
For file upload fields, you can attach images, PDFs, Word documents, Excel spreadsheets, and CSV files. Each file can be up to 500MB in size.
Each form on a job, request, or appointment shows one of three statuses:
If a job or appointment has required forms that haven’t been completed yet, you won’t be able to mark it as done until all required forms are submitted.
From the main Forms page, you can search for forms by name or description. You can also select multiple forms and delete them in bulk using the delete button that appears when forms are selected.

Yes. When you create a form, it’s available to attach to any job, request, or service plan in your workspace. Each one gets its own separate submission, so the same form template can be used across as many as you need.
Existing submissions keep the data that was entered at the time of submission. If you add or remove fields on the form, new submissions will reflect the updated fields.
Forms are designed for your team to fill out on jobs, requests, and appointments. For capturing information from customers, you can use the request form on your client portal or embed a lead form on your website.
There’s no hard limit on the number of fields per form. Add as many as you need to capture the information that matters for your workflow.
Yes. Go to Settings > Jobs & Operations > Jobs for job forms, or Settings > Marketing > Requests for request forms. Any forms you set there will automatically be attached to every new job or request you create.
